dc.description.abstract The Mindanao State University – Maigo School of Arts and Trades (MSU – MSAT) is one of the eleven (11) campuses of Mindanao State University System, located in the Province of Lanao del Norte. As a member of the university system, it has to ensure all its clientele that the faculty can provide quality education with the utmost ability to carry out the tasks and obligations effectively in relation to their students, peers and supervisors as coexisting stakeholders. The university has its mandate to conduct a semestral faculty evaluation based on the MSU System Teaching Efficiency Rating Scale of the 2005 Revise Implementing Guidelines and Procedures (ISRP) approved by the MSU Board of Regents through BOR Resolution No. 211, series of 2006. The scheme is primarily intended to serve as a guide for ranking purposes, productivity incentives and promotions, respectively. MSU-MSAT was one of those learning institutions using the manual process of faculty evaluation every end of the semester, which appears to demand much cost, time and effort, leading to the delay in the submission of reports. Secrecy and confidentiality of ratings and comments are not highly secured. The project was undertaken to build a Local Area Network (LAN) based computerized Faculty Evaluation System. The system was designed using Agile Methodology to guarantee both intended user and developer, prototype was constructed and tested until the project’s objectives were finally achieved. The system accommodates different users; the Administrator as Consolidator, the Encoder, and the five (5) types of Raters; Student, Peer (co-faculty), Department Head, Principal and Dean of Instruction. The students can only evaluate faculty who are his/her subject teacher. The department heads and Principal evaluate only faculty under their direct supervision, while Department Heads and Principals are likewise evaluated by the Dean of Instruction. The computerized Faculty Evaluation System intends; to solve the problems met in the manual process of the faculty evaluation, to ensure that everyone involved can participate, and to generate the most efficient and reliable result of the real performance of faculty.
