Abstract
In general, this project entitled "Service Request System for Bicol Medical Center" aims to address the problems incurred in the manual service request flow used currently by the IHOMP unit of Bicol Medical Center. With the manual system, job/service requests from the different areas of the hospital takes time to be prepared and eventually, processed. There is no systematic way of recording the job/service requests, no way to easily trace who among the staff were assigned for the job and no summary or report of the jobs/services done by the unit. Thus, the manual process makes the performance evaluation of the employee and the office time-consuming to prepare.
In this regard, the Service Request System for Bicol Medical Center introduces an easy way for various offices to easily submit their job/service requests. The system provides a systematic way of recording all the job/service requested and also staffs handling the job/service request. Moreover, the system can generate a report useful during performance evaluation of the staff and office.
To develop the system, a prototyping model was used by the proponent as a guide in developing the system. Technologies such as HTML, CSS and MySQL were utilized to develop the system. After the prototype was created, tests were conducted and the potential users were asked to evaluate the system.
